Forms

We've provided all of the forms you might need in association with your eye exam. If you would like, you can print out and complete the first two prior to your visit. Please bring them along with an identification card and insurance information/card.

If you are a new patient, it is helpful if you arrive 10 minutes early if your paperwork is not completed in advance. If you would like us to have your previous records, there is a record release form. Lastly, the notice of privacy practices outlines our HIPAA compliance policy.
